چکیده

Background Immediate selective hypersensitivity reactions to pyrazolones are thought to be mediated by specific-IgE antibodies. The diagnosis of selective reactions to pyrazolones is mainly based on the clinical history, skin testing and/ or drug provocation test (DPT). Skin testing has low sensitivity and has potential risk of eliciting an anaphylactic response as well as DPT. Therefore the use of in vitro tests could be a safe and straightforward approach, although till now no well-validated test is available. Basophil activation test (BAT) has been proposed as a new in vitro diagnostic tool for different drug reactions although its sensibility is not optimal. The aim of this study was to analyze the potential usefulness of dipyrone metabolites to improve BAT sensitivity.

Pyrazolones metabolites are relevant for identifying selective anaphylaxis to metamizole.

Non-steroidal anti-inflammatory drugs (NSAIDs) are the most common cause of hypersensitivity reactions, with pyrazolones the most frequent drugs inducing selective reactions. Immediate selective hypersensitivity to pyrazolones is thought to be mediated by specific-IgE. Sensitivity of in vitro diagnostic tests is low and this may be due to the incomplete characterization of the structures involv...

Threshold for Positivity and Optimal Dipyrone Concentration in Flow Cytometry-Assisted Basophil Activation Test

PURPOSE Basophil activation occurs both in patients with immediate hypersensitivity reactions to anti-inflammatory drugs and in healthy controls in a dose-dependent manner. Our aims were to define the optimal basophil activation test (BAT) concentration and the threshold for BAT positivity for dipyrone. METHODS From 45 patients with a positive history of an immediate hypersensitivity reaction...

Basophil Activation Tests Based on CD193 Marker in Dipyrone Allergy

414 http://e-aair.org I recently read the article Threshold for Positivity and Optimal Dipyrone Concentration in Flow Cytometry-Assisted Basophil Activation Test by Hagau et al., in the August 2013 issue of AAIR, where the authors assessed the usefulness of basophil activation tests (BATs) in the diagnosis of allergy to dipyrone. Allergy to dipyrone still represents a main concern and pyrazolon...

Inclusion of clavulanic acid determinants in the basophil activation test improves the evaluation of immediate reactions to amoxicillin-clavulanic acid

Background The clavulanic acid (CLV) is nowadays frequently included in the treatment with Amoxicillin (AX). This component of the antibiotic therapy initially thought to have a low immunogenic capacity; however immediate allergic reactions to CLV have been reported in a 30% of patients allergic to AX-CLV. Basophil activation test (BAT) has shown promising results demonstrating specific recogni...

[Basophil activation test in the diagnosis of allergy to medicines].

In this paper we study the reliability of the basophil activation test (BAT) in the "in-vitro" diagnosis of allergy to betalactams and to metamizol, and the sensitivity and specificity of the technique are analyzed. To this end, we studied 58 patients allergic to betalactam antibiotics with a positive cutaneous test facing any derivative of penicillin and 30 healthy controls who tolerated betal...
